AMBULANCE BILLING:

Ambulance billing services are handled by Systems Design West.  General questions about your ambulance bill should be directed to 800-585-5242.

REQUEST FOR BILLING OR MEDICAL RECORDS:

• If you are requesting your own personal medical or billing records for ambulance services you may:
  • Request them in person by bringing a valid, non-expired state or federal ID to 1101 Cushman St. during regular business hours (Monday-Friday, 8am-4pm).
  • Complete a medical records release form, have it notarized, and fax the form to 907-450-6666 (it is strongly recommended that you not use an online fax service unless they specify they are HIPAA compliant.)
• If you are requesting someone else’s ambulance records (medical or billing), then choose the most appropriate option below
  • FOR RECORDS THAT YOU ARE SEEKING ON BEHALF OF A CUSTOMER OR CLIENT
    • Obtain a signed, notarized copy of either our medical records release form or your own (must contain all required HIPAA components) and fax it to 907-450-6666.
  • FOR RECORDS BEING SOUGHT AS PART OF A SUBPOENA:        
    • Serve the subpoena on the City Clerk or the Mayor's Office directly.
  • FOR RECORDS REQUESTED AS PART OF A SEARCH WARRANT (for law enforcement use only):
    • Please securely email an encrypted copy of the warrant to fire@fairbanks.us or fax it to 907-450-6666
  • FOR THE RECORDS OF A DECEASED PERSON OR CONSERVATEE:        
    • Fax a notarized copy of medical records release form, along with a copy of the legal documentation showing that you have the right to access this information (e.g.,  court documents showing guardianship that covers medical care; records that prove you are the executor of an estate…etc) to 907-450-6666.​​​​​​
